Often specifics are undeterminable. But frequently there is helpful information available somewhere. Early reporting is understandably cautious and if no legal actions are taken the press coverage drops off, leaving us without actionable information.
Was this more about raw unpasteurized eggs, raw meat, keeping surfaces clean or a breakdown in hand hygiene? Yes, multiple issues probably play a part but there was likely a single trigger, a point at which the right intervention could have avoided all this pain and suffering.
